PROOF OF CUSTODY (Birth Certificate, Medicaid Info, Court Custody Documents) will be required for all children receiving assistance.

PROOF OF INCOME for all working individuals in the household (Wages, Social Security, Disability, Food Stamps, Child Support, Pension, Unemployment/Worker's comp, assistance form other agencies & any other income.)

PROOF OF EXPENSES for household (Rent/Mortgage, Electric , Gas, Water, Phone, Cable, Auto, Medical, Loans/Credit Cards, Child Care & any other expenses)

ADDITIONAL INFORMATION NEEDED FOR EACH CHILD:

1. CLOTHING SIZES
2. PRIORITY CLOTHING NEED
3. FAVORITE COLOR, CHARACTER, HOBBY OR SPECIAL INTEREST
4. DESIRED BOOK OR EDUCATIONAL TOY

Applications are BY APPOINTMENT ONLY - Appointments will being accepted the following times:

Monday, Oct 21st from 2:00 pm until 5:30pm
