Three Refugees Diagnosed With Active TB in Vermont Over Past Seven Months

A spokesperson for the Vermont Department of Health confirms to Breitbart News that “three refugees [have been] diagnosed with TB in Vermont over the past seven months.”

One case was diagnosed in December 2015, and the other two cases were diagnosed in 2016.
No one other than a refugee has been diagnosed with active TB in Vermont during the first six months of 2016, the spokesperson adds.

The stunning admission comes after Breitbart News reported that the number of active TB cases in Vermont tripled from two in 2014 to seven in 2015.

“In 2015 Active TB Cases were seven. This included one refugee, three foreign born and three U.S. born. In 2016 (YTD) there were two Active TB cases in refugees,” Charon Goldwyn, Chief of Operations, Records Officer for the Vermont Department of Health tells Breitbart News.

But Goldwyn denies that the Green Mountain State is experiencing a TB outbreak.

“In a subsequent e-mail you asked if Vermont is currently experiencing a TB outbreak and the answer is no,” Goldwyn added.

The Centers for Disease Control (CDC) reports that “[d]efinitions for TB outbreaks are relative to the local context,” and that “a TB outbreak is generally defined as a situation where there are:”

More TB cases than expected within a geographic area or population during a particular time period,
AND
Evidence of recent transmission of M. tuberculosis among those cases
Although the time period is not well defined, recent transmission generally refers to a situation where TB transmission occurred within the previous 2-year period.

During the four years beginning in 2012 and ending in 2015, 1,301 refugees have been resettled in Vermont. One Vermont publication reports that 6,300 refugees were resettled in the state in the twenty five years between 1989 and 2014, about one percent of the state’s population of 632,000.

Assuming an estimated refugee population of about 6,300, the annualized rate of TB among resettled refugees in Vermont over the past seven months is approximately 85 per 100,000, high even for a third-world country. This contrasts dramatically with the rate of TB in all of Vermont in 2014, which was approximately 0.3 per 100,000 (two cases out of a population of 626,562 in 2014).

The current rate of active TB among resettled refugees in Vermont, therefore, is approximately 250 times greater than it is among the general population of the state, based on those assumptions.

Internal emails obtained by Vermont Watchdog.org contained information that identify the three resettled refugees in Vermont who have been diagnosed with active TB over the past seven months.

The first refugee was “diagnosed with active TB [and] treated by Dr. Walter Kemper Alston and Dr. Porntip Kiatsimkul, both of the University of Vermont Medical Center.”

Her “treatment began on December 31, 2015.”

The second refugee is “a patient of Dr. Christopher Huston of the University of Vermont Medical Center, whose husband had been treated for active pulmonary TB at a refugee camp prior to arriving in the US, with 3 children, age 13,11, and 7.”

She “was diagnosed with active TB by a GeneXpert test on April 22.” She “started on the four drug protocol earlier in the month.”

The third refugee is “a patient of Dr. Krystine Speiss and Dr. Alston, both of the University of Vermont Medical Center.”

This patient’s treatment began around May 27, the date Vermont Department of Health public health nurse Sydney White said in an email that “Her CXR was OK so they plan to have her do daily INH/RIF/B6 for 9 months, no PZA or EMB. Kemper said she should self administer since she is extrapulmonary.”

Darcie Johnston, founder of Vermonters for Health Care Freedom, finds the dramatic recent increase of active TB cases among recently resettled refugees in Vermont alarming.

“With all the state’s focus on the ‘epidemic’ of opiate abuse, in order to justify all kinds of draconian intervention into the patient-physician relationship, we shouldn’t forget about traditional epidemic diseases, and we should wonder why cases like this don’t provoke draconian restrictions on refugees,” Johnston tells Breitbart News.

“On Friday, the Vermont Department of Health announced this year’s first human case of West Nile Virus. Vermonters have a right to know all public health threats and no government has the right to keep this information from the public. Vermonters for Health Care Freedom call for a full disclosure of how many TB cases are in VT, if all are under treatment and who is paying the cost of treatment,” Johnston adds.

The Vermont Department of Health on Thursday answered the first of Johnston’s questions, but the remaining two—if all TB cases in Vermont are under treatment and who is paying the cost of treatment—remain unanswered.

Vermont Admits 17 Refugees Diagnosed With Active TB

After Breitbart News reported that three refugees in Vermont have been diagnosed with active tuberculosis (TB) over the past seven months, an official with the Vermont Department of Health admitted that a total of fifteen refugees in the Green Mountain State were diagnosed with active TB between 2003 and 2015.

That brings the total of refugees the Vermont Department of Health now admits have been diagnosed with active TB between 2003 and June 30, 2016 to seventeen.

One of the three diagnosed in the past seven months was diagnosed in December 2015. The other two were diagnosed in 2016.

On Thursday, Vermont Public Radio reported on the data it received from the Vermont Department of Health on rates of tuberculosis in the Green Mountain State:

Between 2003 and 2015, the state has recorded 77 cases of active tuberculosis in Vermont. Of those, the Health Department says just 15 were diagnosed in refugees. 26 of the cases were in U.S.-born individuals. The remainder were in people who fell into other categories, like people living in Vermont on student or work visas.

Since only 6,300 refugees have arrived in Vermont over the past twenty-five years, these new statistics reveal a third-world level of active TB among the refugees currently residing in the Green Mountain State.

During the thirteen and a half year period between January 1, 2003 and June 30, 2016, 17 of the 6,300 refugees in Vermont were diagnosed with active TB, or one out of every 370 refugees. During that same period of time, 26 of the average 620,000 U.S-born residents of the state were diagnosed with active TB, or one out of every 23,846 U.S.-born residents of the Green Mountain State.

Refugees developed active TB in Vermont at a rate more than 60 times greater than U.S.-born residents.

Shockingly, Vermont State Epidemiologist Patsy Kelso, who has a Phd. in Public Health from Johns Hopkins, told Vermont Public Radio on Thursday this high rate of tuberculosis among refugees is “not a concern of mine as the state epidemiologist.”

“We will continue to see refugees with latent TB infection [LTBI] and we will continue to see, in rare cases, active TB illness in refugees – like we do all the time in U.S.-born Vermonters,” Kelso added.

Kelso’s statement is one of the most egregious examples yet of the politicization of the public health establishment in the United States at the expense of science and the public health.

When one out of every 370 refugees in Vermont develop active TB over a thirteen and a half year period, it is simply factually incorrect to declare those cases to be “rare,” or to suggest they occur at the same rate as active TB during the same time frame among U.S-born residents, when it is 60 times greater.

“Roughly a third – about 35 percent I think it is – of refugees test positive for TB, meaning they have latent TB infection . . . And it varies a little bit. It depends on what part of the world they come from, because there’s more TB in some parts of the world than in others, ” Kelso said, failing to note that Vermont has the highest rate of LTBI of any state so far reporting this data among refugees.

As Breitbart News reported, 29 percent of refugees in one county of North Dakota test positive for LTBI.

The nearest state to Vermont in terms of percentage of refugees who test positive for LTBI is Tennessee, where 27 percent of refugees are infected.
The LTBI rate among the general population in the United States is only 4 percent.

Though on average, only 10 percent of those with LTBI go on to develop active TB, the high rate of LTBI among refugees “increase[s] risk of active TB in refugee resettlement communities,” according to a 2013 UC San Diego study.

As Breitbart News reported previously, the Vermont Department of Health has yet to provide any data on the percentage of refugees in the state who test positive for LTBI and successfully complete the recommended treatment program.

The 2013 UC San Diego study found that completion rates for LTBI treatment among refugees are often low, thereby heightening the health risk those with LTBI pose to the general population.

In her Vermont Public Radio interview, Kelso also claimed, “There’s a really stringent medical screening that refugees go through overseas.”
“[A]ll refugees are required to get a chest X-ray before coming to the U.S,” Vermont Public Radio reported she told them.

“If that chest X-ray indicates possible TB, they undergo further testing, including lab tests. If they’re diagnosed with TB, they’re not allowed to come here until they go through six months of treatment and further lab testing to show that they’ve been cured of TB. Only then are they allowed to come,” Kelso added.

Kelso ignores the simple fact that most, if not all, of the seventeen cases of active TB appear to have come from refugees who were allowed to enter the United States with latent TB or what the State Department calls “active non-infectious TB,” perhaps one of the greatest oxymorons ever used.

In other words, they easily passed the so-called “stringent medical screening that refugees go through overseas” despite pre-existing conditions that indicated tuberculosis risk, came to the United States, and developed active TB here.

Refugees diagnosed in overseas medical screening with active extra-pulmonary TB may not be infectious.

But there’s another group–those with positive chest x-rays that indicate active pulmonary TB who are “sputum smear negative” but who did not receive the more finely tuned smear culture tests that can detect the presence of active TB that smears miss–who are classified as “active non-infectious” and allowed to enter the United States even though they may be infectious under the “B-1” tuberculosis health risk classification.

Some Vermont residents are tired of the constant stream of propaganda coming from the state’s public health establishment and the complicit refugee resettlement industry.

“It’s time for public health officials and politicians to stop the propaganda and admit that the refugee resettlement program has turned the United States into a treatment center for people from other countries who have or are likely to develop active TB, all at the expense of the American taxpayer, at both the federal, state, and local level,” Darcie Johnston, founder of Vermonters for Health Care Freedom, tells Breitbart News.
