My local office did not have any, so Washington DC will send me ten forms. You can also download them from the IRS website. For those unfamiliar with the category, these are used to make a complaint against a non-profit organization granted tax exemption as a non-profit--commonly called a 501c3 exemption. They are not used to report PAC's.

Religious groups operating with 501c3 exemptions must refrain from illegal activity, excessive lobbying, terrorist activities, and partisan activity-among some other things. Do you know of any religious groups that might be doing things like this? I called on the "IRS forms" phone number in the US Government listings. I can't see why "partisan activity" would only be limited to Dems and Reps; aren't parties like Vicente Fox's PAN party poltical parties, too? (hint, hint). So churches that wholeheartedly assist with PAN's foreign policy program for the US might be violating that restriction. No?
